Yup, or a screw with a stripped head. Had one of those just a little over a week ago here on something and I was danged glad I had a set of screw extractors.

The Ampex team that developed the VRX-1000 helical scan VCR VTR (no videocassettes in those days).
One of those guys is Ray Dolby, and another is Fred Pfost. I worked for Fred Pfost's son for a while in a past life. I had the privilege of spending a very pleasant afternoon with Fred Pfost & the aforementioned son at Fred's home in Los Gatos some 30-ish years ago.
20th anniversary photo of the VTR and (some of) the team (1976).

It's also, perhaps, worth mentioning that a number of companies had developed stationary head video tape recorders prior to the VRX-1000. In order to get the bandwidth required for video recording (ca. 4.5 MHz), the tape velocity on such recorders was terrifyingly high.

RCA's early model operated at (up to) 360 ips (!)
