PSW404 no output

I am getting no output at all from my PSW404. I have a new blu-ray player and was connecting as an external decoder through my Yamaha surround receiver and noticed no woof. Then I tried the optical connection and selected the DVD/CD input and have same problem. Prior to the blu-ray, I was just running the sub off the leads from my RM6700 system. This was probably a problem prior to the blu-ray as I haven't used my system in a while. I have tried running a direct cable from the DVD's sub out to the LFE input, a direct cable from the receiver's sub out to the LFE, as well as the original RM6700 setup and nothing. Green power light is on. I can't see a fuse holder on the back of the sub, so am not sure where to go from here. How can I test the PSW to ensure it's not just dead? Help appreciated.

    Try using the speaker level inputs, if that dose not the help contact Polk's support line

    Do you have the sub turned on in the blu-ray player ? If so, adjust the sub volume all the way up in the player. Crossovers set ?
