RM6750 Sub not working again

this is now the second time that my sub has stopped working. Not sure what is happening but need to get it repaired and also figure out what is causing this.
Any thoughts?

Sorry you're having a problem. Can you describe what happened? Is the bass amplifier's LED illuminated?

yes led is on. already checked fuse and its fine.
noticed it stopped working during some music. same thing happended last time. i sent in an email to polkcs already as well. was repaired but never found out what happened to it. here is Sales Order: 699554 for your reference -
