Db6500 blown woofer

I had all my speakers replaced less then a year ago and one of the woofers is toast, (I had it looked at), the other 3 work just fine, can I send it in to be fixed, how do I go about this process? DB 6500 is the series I have installed

    Hello,
    Usually this is covered in the warranty section of the owner's manual. You, most likely, will send the blown woofer to the San Diego address and include a copy of the sales receipt. The address is on Polk's web site, 2550 Britannia Blvd., Suite A, 92154.
    Then you have to look at your system and find out why it blew. Otherwise it will happen again.

    I had my system installed by circuit city. I had to go back to them a few times becasue of problems with the work they did. The first time they didn't put the covers on the crossovers and the speakers stopped working, when they opened the door frames they found the broken insides to the crossovers all over inside the doors. They replaced them and very soon after the woofer stoped working so i took it in again and they said it was dead and I would have to get a new one. I have a 4 chan. amp in the trunk, i thought this would help the speakers to not blow, i rarely play my music loud. I think Circuit City sucks, i do not have the recipt to my speakers though. Should I just call customer care or should I just send it in to get fixed?
