SB1+ turns off after 5 mins

Hello,
I have a Polk Audio SB1+ connected direct to my TV via optical cable.
It seems to disconnect itself then shut off after 5 mins as well as stops pairing with the sub woofer.
The usb blue tooth indicator LED and green sub woofer LED both begin to blink.
On the front side of the bar a single white LED located on the far right and left to the green LED keeps flashing.
The unit is the main source for audio. When its connected the TV volume controls are disabled.
We primarily use Roku for streaming.
A few things I've tried so far.
- reset power and unplug both sub and bar completely
- unplug all cables
-Attempted factory reset, unclear if it worked.
-While setting up the device with the Omni App, I connect to the playfi wifi signal, but the unit shuts down before I can get anywhere.
There isn't much information on the internet, any Ideas how to fix?
