Polk Audio PSW125 subwoofer distortion sound and extremely low volume

Hey Everyone,
Need to get some help/advise from you guys.
My PSW125 was out of order. It is now buzz for a few seconds when you connect to the power. After that I can still hear some noise without connecting to any audio source intermittently.
If connect it to a audio source, it generates distortion sound with extremely low volume.
I have recorded the sound and uploaded it into Youtube.

Woofer was measured by a digital Multi-meter - It shows 4 ohm consistently.
1) Is the issue on Amp or Woofer?
2) If the problem is on Amp, would capacitor change solve the issue?
Any comment are welcome!
