Command Bar / Random Streaming

On two different occasions my Command Bar has decided to play music of its' own choice with being commanded to play. A couple of weeks ago while watching TV the Command Bar started streaming music. I pressed the HDMI 1 button and the TV came back on - for a few seconds - and then the music started over again. I went through this process a few times and it continued to switch back to music on its' own. I had to unplug the command bar to get it to stop. This morning I was awaken by music on the Command Bar. I thought that maybe I had left it on from the night before and I asked it to stop and it did - for a few seconds - and started right back up. It was playing music that I would have never selected in both instances. Have I been hacked or is there an issue that needs to be dealt with? I really don't think I've been hacked or I would have issues with my other Amazon Echo Dots.
