SurroundBar SDA IHT Subwoofer won't stay connected.

Hey guys,
I have an original SurroundBar SDA IHT (manual: http://www.polkaudio.com/downloads/manual/SBSDAIHT_MN.pdf)
in the last 4 or 5 months, the subwoofer will disconnect, and the lights will flash on the bar and the back of the woofer.
If i power cycle the woofer, it will connect right away, it lasts about 5 seconds before cutting in and out, then eventually loses connection again within about a minute.
I've tried all 4 channels, multiple times. Loud or quiet, the problem is the same.
Any thoughts?
