Problem with new Polk Audio PSW10 subwoofer

I'm setting up a new system and am having problems with my new Polk Audio subwoofer. The sub produces sound, but it's very low. When I run the auto config on the receiver, it gives me an W-3 error...which basically shows the subwoofer level is very low.
It's a Polk Audio PSW10 sub and it's hooked up to a Yamaha RX-V471BL. I have the sub connected with a Mediabridge Ultra Series - Dual Shielded Subwoofer Cable which plugs into a Monster ILJRY-1 F Single Female RCA to Dual Male RCA Y-Adapter. The adapter is plugged into the L & R jacks on the sub.
I'm a newbie to anything besides HTIB systems, so maybe I'm doing something wrong. I tried plugging the sub cable directly into the L input without the adapter, but got the same result. Could it be a bad cable? I have checked to make sure the receiver is set to use the subwoofer.
Does anyone have any ideas on what I can try? Thank you!

What level is the sub set at now?
