I am trying to listen to a music DVD without having my TV on. The HDMI cable runs from the bluray directly to my AVR and then monitor out to the TV.
Why does the TV have to be on? I could see if it was the other way around.

It shouldn't. My guess is to see if your TV is set to do some sort of HDMI control interface with the receiver, like Panasonic's Viera or Samsung's Anynet. Try unplugging the HDMI from the monitor out on your receiver then play the DVD. I bet it works.

Actually, it more likely has to do with HDCP, which is used for copy protection. It is known to cause all kinds of annoying issues, including this one. Unfortunately, the only real workaround is the one Zeros suggested.

Those are the cool advice nuggets you get from guys on this site, no telling how far and wide you would have had to search, I mean really.
