12 o'clock on an AVR

I've seen in some previous threads not to push certain AVR past 12 oclock on the volume and I understand the terminology on an older receiver in that you don't put the volume line past the 12 oclock position when you have a volume dial, but when you're dealing with a digital display where you have no line on the volume control, what is considered 12 oclock? is it 0db or less or more?

I know you don't want to play too loud when breaking in speakers.. how loud is too loud? is it over 0db or is it closer to 90db when extended listening can start damaging your hearing? just curious I haven't had my M60's long and I don't want to damage them by playing it too loud but i'd like to know where that too loud spot is..

    I have had my pioneer all the way to 3+ without a problem and had it there for about 3 songs. Just listen for distortion if you hear any back it off or if you are in doubt back it off.
