Finding maximum theoretical dBs

Ok, I know that if sensitivity is known along with the rated output of an amp that the theoretical loudness the speaker can achieve can be found. I just don't know what the formula is for finding out.
Also, what dictates the sensitivity of a speaker? The driver, cabinet or both?

SPL goes up 3db every time the power doubles.
SPL goes down 6db every time the distance from the speaker doubles. (sensitivity is rated at 1 meter)
SPL goes up by 6db if # of speakers are doubled. (i.e. - 2 woofers in the same box (SVS /2 subs), two speakers next to each other, etc.)
Room boundries can factor into it as well, but not sure how to calculate it.
