A/V Wall Plates

I'm currently in the process of rebuilding the entire wall around my fireplace and would like to put a plasma over it. I was hoping to put in jacks in the center of the wall and run them down to jacks on the shelving next to it. This way I can easily hook up things as time goes on (video games, cameras, ect).
I wanted to be able to run a jack for ever input on the TV (component, composite, HDMI, S-vid, audio and digital audio). The RCA jacks are easy, but I can't seem to find anything for HDMI or S-video. Does anyone know where I can get these? Or does anyone know what the best way of building a large plate with all those connections (instead of a bunch of standard wall plates)?
(I'm extreamly handy and willing to take on any sort of project).
Thanks.

Hello friend, here are some sources you might try. I have used them all and recommend them equally for reliable and fast shipment:
mcminone.com
partsexpress.com
matelectronics.com
Good luck with your installation! -
I did some searching myself as your HDMI got me thinking...
Found this looks pretty cool, as I found nothing else expect HDMI plate and nothing else. This looked like you could built something just for want you needed nothing more / nothing else.

Thanks for the help guys. I think I'm going to go with parts express. They seem to have almost everything at a pretty good price. Thanks for the offer on the s-vid jack, but I should be able to ge one there for only a few bucks.
For the HDMI, I think I'm just going to run the cable through the wall and dangle it out the other side. (It's just to much $ for now to have an outlet on the wall [with an outlet I would need another HDMI cable])
I checked out that custom built one, and it's a little on the pricey side. I'm trying to keep this a little on the cheaper side for now.
thanks again.Denon AVR-3803

This is probaly a last resort but if you can't find the wall plates you want, this is where I ordered mine from. They are expensive but they have everything.
