My guess is CS400 (i). Are you SURE only one midbass is working? They are crossed over differently, so that one driver only works up to 800Hz or so. This often makes it seem that only one driver is working, because one is obviously louder than the other one. It's fooled a lot of people, so you wouldn't be the first.
If you're interested, here's basically why they do this: If both drivers try to play the same frequency, and the wavelength of that frequency is less than the distance between the drivers (wavelength goes down as frequency goes up), it causes "comb filtering" effects that result in a series of peaks and nulls in the horizontal off-axis response. Since a center speaker is generally oriented horizontally on or below a TV, and not everyone can sit directly in front of it, they design the crossover this way to produce a wider "sweet spot".
You could always open it up and disconnect the driver that you know to be working, and that way you can tell for sure if the other one is working or not.

Oh, and if it actually is not working properly, a call to Polk CS would be the first step to take. I think the midwoofers are still available, and if you have to buy one you'll get a better deal if you tell them you are a Club Polk member.
