If no one does, go to the bargain bin at your local CompUSA or something, they usually have em for like 5 bucks.
Or you can get this , shipped to your door, for under ten.If you will it, dude, it is no dream. -

Since I don't know the age of your 'putie, do you need one for an ISA (long black slot) or PCI (shorter white slot). I wouldn't pay that much for a modem either. If iI were in the market, I'd go for an OEM without a fancy box.
