Need help identifying this amp

My car was broken into and my front MOMO speakers and Pold amp were stolen. I'm looking for my records, but its been a while since I had this installed. Meanwhile, can anyone help me identify this amp?

    Not sure about that one bud. I date back to the Carbon series amps and that one must be before that time cause I dont recognize it at all. Might try Googling Polk amps and see if there are any pictures of that one with a label to it.
