Polk Monitor 5 Tweeter ID

I picked up a pair of Polk Monitor 5's last week to go with my Polk 10's and 4's.
It looks like they have a replacement tweeter but I cannot identify it.
I pulled it out and it had the following info on the back of the tweeter;

    What does the front of the tweeter look like?
