OK, so I honestly don't know alot about this amp other than it sounds very good. I'm replacing it with a much more expensive full tube OTL amp, so this one needs to find a new home.
There is plenty of talk about it over at Head-Fi, and it will probably be easier to sell there, but wanted to give you guys here a shot first as I'm not very active over on that other forum.
Here's a link to the DIY page with more info - http://www.diyforums.org/MiniMAX/MiniMAXoverview.php
I did not build this one myself, but purchased from someone here on the Polk forums. I suspect that if Keith sees this thread he'll probably chime in with his thoughts on this amp as he used it for a while.
I'm just looking for a balanced tube only setup and have my eye on a couple models by Woo Audio, and this one needs to be sold to help fund that purchase.
I don't have pics handy and my camera battery is dead, but mine looks like the one in the link above, except that it has a very cool clear see through top so you can see all the guts. It comes with 2 sets of tubes, the currently installed and biased 12AE6 high gain tubes and a pair of 12FK6 low gain tubes.
$200 get it shipped anywhere in the lower 48. I'm charging the camera battery now and will take pics when that's up.

Pete Millett's headphone designs are excellent at all price points... he is a pro designer and erstwhile for-profit headphone amp manufacturer, and also a great friend to the DIY community (http://www.pmillett.com/) par excellence.
He also maintains a tremendous on-line resource at www.tubebooks.org
All of which is by way of saying - this should be a very nice, high-value headphone amplifier! :-) -
