Has anyone ever had a defective amplifier, or one go bad or heard of someone who has had this problem with either a polk subwoofer or powered tower.
The reason I ask is I am wondering if it would be worth getting an extended warranty for a powered tower and subwoofer.

the polk warranty already cover's the amplifier for 3 years and the loudspeaker for 5, so IMO, an extended warranty on polk speaker's doesn't really buy you much...(that is if you bought from an authorized dealer)
